Clinical Audit: The End of Project Report

The report should be retained in your Clinical Governance portfolio.

The report should be laid out as follows:

Introduction
To include
  • Title of project, and subject to be audited
  • Aims and Objectives
  • Design of project
  • Details of discussions with facilitator
  • Standard Set
  • Details of source materials and references

    Method
    To include
  • Method of data collection
  • Example of data capture form(s) used
  • Data analysis, with details of the sample size

    Discussion of results

    Conclusions
  • Did the project meet its aims and objectives?
  • How did the results compare to the standards set in the original bid?
  • What problems were identified?
  • What educational needs were identified?
  • A detailed timetable of activity to allow the LAP to assess the number of hours the project has taken, for probity purposes

    Action Plan to implement improvements
  • How will change be planned in the future as a result of the audit?
  • What steps will be taken to address the educational needs?
  • Time scale for revisiting the audit
